INSTRUCTIONS FOR INCOMING FRESHMEN
1st Semester, 2011-2012
Step 1. Submit yourself for a physical-medical-dental examination.
All freshmen are required to undergo a complete physical-medical-dental or health check up at the CLSU Hospital. You are expected to report for this examination on the DATE and TIME indicated in our letter.
You will be asked to pay eighty pesos (P80.00) for this examination.
Get the results. If you are issued a medical certificate reflecting your fitness to study in CLSU, you have satisfied another admission requirement.
Step 2. Secure lodging permit.
Proceed to the Office of Student Affairs or OSA (beside the CLSU Hospital). You will be asked to present your Medical Certificate. If you will be accommodated in the University dormitory, you will pay the dormitory fee that ranges from P900 to P1,250 per semester.
You will be issued your lodging permit.
Step 3. Apply for admission to the concerned Department/College (Unit) if you intend to enroll in any of the following courses (SKIP Step 3, for other courses):
